Below is information on immunization requirements for your school age student.

If your student needs an immunization and he/she does not have private insurance or CHIP, your school nurse would be able to administer the required immunization (s).  The following forms may be downloaded, filled out and returned to your school nurse giving permission to adminster the required immunizations:

• child vaccines-eligibility form
• immtrac  consent form
• consent to give immunization
• Screening Questions

If your student has insurance, you will need to take him/her to their doctor and or health care provider.
