COVID PFIZER Vaccines Offered for SISD Students 12 and Older

Smithville ISD Parents/Guardians,
On May 12, 2021 the CDC approved the FDA recommendation to allow the Pfizer COVID-19 vaccination to be offered to individuals 12 years of age and older. Smithville ISD has agreed to allow APLMG to offer free Pfizer COVID-19 vaccinations to students who are 12 years of age and older, at a vaccination clinic to be conducted at the SISD Junior High Campus on May 27, 2021 from 1:00-3:30 PM.
Parental/Guardian permission is required for APLMG to administer the shot to children 12-17 years old. Parents are not required to be present for the vaccination.
Parent/guardian permission can be given as follows:
Complete and return the attached Vaccine Administration Record (VAR) form for each child you would like to be immunized. BE SURE to complete the signature lines above Section B-1, Section B-1, Section B-2, read and sign section B-3. Return the completed VAR to the JH or HS Nurse’s office by Monday, May 24, 2021. This is a firm deadline.
Some SISD HS students received their first COVID-19 vaccination of Pfizer at an APLMG clinic held at the High School on May 10, 2021. Those students could receive their second shot on May 27, 2021.
A clinic for students needing their second COVID-19 vaccination will be held on June 17, 2021 at Smithville Junior High from 1-3:30 PM. This clinic will be for any students receiving their first shot on May 27, 2021.
